Trauma and suppressed emotion shape addiction and disease

Gabor Maté (born January 1944) is a Canadian physician and author known for his work on trauma, addiction, stress, and the mind-body relationship. Born into a Jewish family in Budapest, Hungary — his maternal grandparents were killed at Auschwitz when he was an infant — he survived the Nazi occupation and emigrated to Canada in 1957.

After twenty years in family practice and palliative care, he spent over a decade working with severely addicted and mentally ill patients in Vancouver’s Downtown Eastside, an experience central to his influential book on addiction, In the Realm of Hungry Ghosts (2008).

Maté’s core thesis, developed across When the Body Says No (2003), Scattered Minds, and The Myth of Normal (2022), is that emotional experience — especially early trauma and chronically suppressed feeling — shapes physical and mental health, contributing to conditions from addiction to autoimmune disease.

He is also the originator of Compassionate Inquiry, a psychotherapeutic approach now studied by practitioners in many countries, which works to surface the hidden beliefs, implicit memories, and body states beneath a person’s presenting story.

His trauma- and attachment-centred reframing of addiction and his insistence on treating the whole person are widely respected and have shaped trauma-informed care. Some of his stronger claims — that trauma or stress is a primary cause of illnesses such as cancer — are contested, and reviewers note these extend beyond what the current evidence establishes; the well-supported core is the real influence of chronic stress and adversity on the body, mediated through known physiological pathways.

Reframing addiction as a response to trauma: Shifted the understanding of addiction from moral failure or mere brain disease toward an adaptation to early pain, foregrounding compassion and underlying emotional wounds.

  • Articulating mind-body causation in illness: Popularised, in When the Body Says No, the link between suppressed emotion, chronic stress, and physical disease, drawing on the physiology of stress.
  • Compassionate Inquiry: Created a therapeutic method that works directly with the subconscious patterns, implicit memories, and felt body states beneath conscious narrative.
  • Mainstreaming trauma-informed thinking: Helped bring trauma, attachment, and the emotional roots of suffering into broad public and clinical conversation.
Resentment is soul suicide.
When the Body Says No (2003)
Much of what we call personality is not a fixed set of traits, only coping mechanisms a person acquired in childhood.
When the Body Says No (2003)
For those habituated to high levels of internal stress since early childhood, it is the absence of stress that creates unease, evoking boredom and a sense of meaninglessness.
When the Body Says No (2003)
Emotional competence is what we need to develop if we are to protect ourselves from the hidden stresses that create a risk to health.
When the Body Says No (2003)
Not all addictions are rooted in abuse or trauma, but I do believe they can all be traced to painful experience.
In the Realm of Hungry Ghosts (2008)
The question is never “Why the addiction?” but “Why the pain?”
In the Realm of Hungry Ghosts (2008)
Trauma is not what happens to you but what happens inside you.
The Myth of Normal (2022)
Disconnection in all its guises — alienation, loneliness, loss of meaning, and dislocation — is becoming our culture’s most plentiful product.
The Myth of Normal (2022)

Taught by / Influenced by: psychoanalytic and attachment theory, Hans Selye’s physiology of stress, and Buddhist and mindfulness practice, all read through his own clinical experience and personal history of early trauma.

  • Taught / Influenced: the trauma-informed therapy movement, the global community of Compassionate Inquiry practitioners, and contemporary approaches to addiction recovery.
